Every price a maker charges a patient who pays without insurance through its own pharmacy, per fill, by dose; the statistics count the standing prices, not first-fill offers or late-refill prices. Below list is (list − price) ÷ list, rounded to a whole percent, where the maker publishes a list price for the same package.
Maker price changes
Every change to a maker's self-pay or list price since each product came to market, with its source: 42 changes, the newest on September 1, 2026.
Full index
One row per product, 12 in all: the list price (WAC) per package and the cash price a month as each program states it, with the page each was read from and the day.
The list price is per package: 28 days for pens and vials, 30 tablets for tablets. The cash price is per month, as the program states it: a range is a dose-tiered price, and “from” is the lowest entry price. Compounded products have no maker and no list price.

Method
- List price (WAC)
- Wholesale acquisition cost: the price the maker publishes to wholesalers, per package, before discounts and rebates. It is not what a patient pays.
- Cash price
- What a person with no insurance pays for one fill through the maker’s own pharmacy, as the maker states it, by dose. A year is the standing price times the fills in a year: 13 four-week fills or 12 thirty-day fills.
- Program index
- The all-in monthly price (the plan, the medication and any membership) of each program in our program register that sells one drug with the medication included, one program per company. Compounded drugs are not FDA-approved: the FDA doesn’t review their safety, effectiveness or quality before they’re sold.
- Maker self-pay index
- Every price a maker charges a patient who pays without insurance through its own pharmacy, per fill, by dose; the statistics count the standing prices, not first-fill offers or late-refill prices.
- Statistics
- A program index needs at least 3 programs. Quartiles and the median use linear interpolation between ranked prices (Hyndman and Fan type 7, as in R, NumPy and spreadsheet PERCENTILE.INC): the median of an even count is the mean of the two middle prices. Every statistic is rounded to the cent. The mean is the plain average. The weekly history records each index as it stood at the end of each week, on the Sunday that ends it.
- Sources
- The makers’ own pages: Novo Nordisk for Wegovy (list price), Ozempic (list price) and Rybelsus; Lilly for Zepbound (list price), Mounjaro (list price) and Foundayo (list price). Program prices come from the register, and each program links to its review, where its plans and check dates are listed.
- Inclusion
- Every brand-name product the makers sell in the United States (Wegovy, Zepbound, Ozempic, Mounjaro, Foundayo and Rybelsus), and every program in our register with a verified all-in monthly price, medication included. A program index needs at least 3 programs. A price goes in only when we can read it on a public page; we do not estimate a missing price.
- Cadence
- Every week we re-read each maker price from the page it cites. When the page still shows the same price, the check date moves; when it shows a new one, a person checks it before the row changes, and the change goes in the log. Program prices update with the register, and each program index is recorded at the end of every week. On the first day of each month the full index is frozen as a dated edition that never changes. The newest maker check was October 26, 2026 and the oldest still in the index October 25, 2026; the newest program check was October 26, 2026.
- Limitations
- Many prices are tiered by dose; program indexes use each program’s cheapest plan, which is often the starting dose or a plan billed for several months. Prices exclude insurance copays, net prices after rebates and Medicare or Medicaid prices: for those, use the cost calculator. The index does not include NADAC yet. Every figure is true on its check date only.
